The library is offering you the opportunity to purchase a book for the library in honor of your child’s birthday. A book plate will be placed in the front of the book stating the child’s name and birth date. Your child will be presented the book during class time on his or her birthday. After he or she has read the book, it will be returned to the library for others to check out. Your child’s name and picture will appear on the BIRTHDAY BOOK CLUB board in the library. Please send a small photograph or snapshot for this purpose. Your child will also receive a special invitation to attend a Birthday Book Club ice cream party at the end of the school year.

Please consider this worthwhile project and return this form with your $10.00 donation, cash or check. This is a tax-deductible contribution. Your child will be able to choose his or her book from a large selection if you return this form during the first two weeks of school.
